Our Process
for Maximum Impact

Intake & Assessment
Youth and families receive a comprehensive psychosocial assessment to identify strengths, challenges, trauma history, safety concerns, and immediate needs. Families then work with social workers to develop individualized support plans.

Resource Coordination
YouthConnect connects families to community resources like food access, housing support, healthcare, job opportunities, educational services, and emergency assistance.

Clinical Case Management
Social workers organize care across schools, the juvenile justice system, probation, mental health providers, community organizations, and public agencies to give families consistent, responsive support.

Therapy Services
We provide ongoing therapy at home, in schools, in community settings or through telehealth, addressing trauma, substance use, emotional regulation, family relationships, and more.
